Please complete the following form to place your order of donated (no-fee) turkeys. Since we only have a limited number of cases, each active agency in good standing will be eligible to receive up to six cases of turkeys (24 turkeys total). The Agency Relations team will confirm your requested turkey order with you prior to the order being placed.
All TEFAP agency sites agree to have prominently displayed in clear sight of recipients the following signage at times whenever commodities are distributed:
a. “And Justice for All” poster Form AD-475A
b. Notice to the Public
c. No Selling Sign
d. Current CDSS established Income Guidelines next to the EFA-7
e. TEFAP Written Notice of Beneficiary Rights
f. No Application sign (This only applies to agencies that request an application for other food and/or services)

TEFAP participating agencies are required to register and obtain the Unique Entity Identification through SAM.gov. Agencies that have not completed the request will not be placed on hold as of now. However, your agency must be actively working towards obtaining the required unique entity identification.
All agencies must apply for their own Unique Entity Identification, even if your agency is part of a 501(c)3 umbrella i.e. SDA’s, Catholic Conference, Salvation Army, etc.
Remember when requesting your agency’s UEI to use the name of your organization and your agency’s address.
If during the validation process, you are required to submit the date of incorporation and state of incorporation. These can be found by searching your agency’s full business name on the Secretary of State Business website (https://bizfileonline.sos.ca.gov/search/business)
Once your agency has completed the registration, kindly fill out the following LARFB - UEI Agency Form to provide your Unique Entity Identification (UEI). If your agency did not make the record public, please use the LARFB - UEI Agency Form to upload a copy of UEI registration after saving the file and/or taking a screenshot.
This Quick Start Guide is a step by step process on how to register/obtain the Unique Entity Identification.
All TEFAP Agencies will be required to submit their UEI information by December 31, 2024.

Update for Extra Helpings Agencies
Agency Partners, We have revised the agreement to reflect the following updates:
Distribution Schedule: Agencies must distribute food weekly.
Temperature Guidelines: Stores are increasingly concerned about the safety of transported food.
On-site Records: Agencies must maintain Temperature Logs and Donation Receipts for both Monitor Visits and Audits.
MealConnect Entries: Entries must be made within 24-48 hours unless your agency has been approved for a weekly entry.
Records Provision: We may request records as part of our ongoing audits of participating agencies.
Extra Helping Agencies, if they have not already, must submit their Extra Helpings Agreement by Monday, October 14th. If you have any questions or concerns, please email extrahelpings@lafoodbank.org.
Please note that failure to submit the agreement by the deadline may result in the termination of your Extra Helpings partnership. We will not accept screenshots or photos of the agreement.

Their website's main aim is to help non-profit organizations or initiatives by translating texts or websites free of charge and to help translators who may be starting, continuing their careers, or re-starting completely. As long as their requirements are met, the PerMondo initiative offers non-profit organizations a free translation service for websites and other texts.

Arrive at the Food Bank Ready with ID Cards & Insulated Blanket
All agency representatives picking up orders and/or shopping must present an agency identification card and insulated blanket to the Food Bank Personnel. There is a $5.00 fee for replacement cards. The charge will be added to your agency’s account.
Dress Appropriately
There are no open toed shoes or open heeled shoes allowed in the warehouse.

Civil Rights Training is mandatory training for all staff of volunteers at TEFAP Sites who interact with participants. The Food Bank Agency Relations Team offers this training monthly.
